Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule. Everyone at our office is at your service. Consultations are available by appointment.

Office Appointment Policies
All appointments must be confirmed 24 hours in advance as unconfirmed appointments are subject to cancellation. Texts and emails are sent to patients prior to appointment. Text messages are sent at least 3 weeks prior to appointment, 2 days prior, and same day of appointment. Emails are sent 3 weeks prior, 3 days prior, and same day of appointment
If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy on your part will make it possible to give your appointment to another patient.
The following forms can be downloaded to your computer and printed at home. Please fill these forms out prior to your appointment and bring with you. It will save a significant amount of time during your visit and will ensure you have all of the information that our staff will need. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with any questions on the day of your visit.
